Medical Repatriation from Aruba to Canada: How It Actually Works, Cost, and Options

When a Canadian is hospitalized in one of the two hospitals in Aruba, the first priority is always stabilization. Aruba’s health care standards are high compared to many other Caribbean islands, and emergencies are generally handled appropriately.

But once the immediate crisis is under control, families usually start wondering how to get home.

Medical repatriation from Aruba to Canada is more common than people realize — especially during snowbird season. The good news is it can usually be arranged safely within a few days once the patient is medically stable.

Here’s how the process looks like.

Step 1: Medical Stability First

No airline will accept a patient without medical clearance and proper preparation for mobility limitations or in-flight care needs.

The treating physician in Aruba must confirm the patient is fit to fly. This includes:

  • A detailed medical report
  • A fit-to-fly certificate outlining what is required for safe transport, and the level of care needed.

Although medevac aircraft from various countries land on the island daily to pick up tourists, most patients do not require an air ambulance.

Subject to conditions, a commercial flight with a medical escort is often the most comfortable and cost-effective solution.

Step 2: Choosing the Right Transport Option

There are two options to transfer a patient from Aruba to Canada:

Commercial Flight with Medical Escort

For patients who can tolerate sitting upright, mobility assistance and in-flight care can be arranged on a scheduled commercial flight. Ideally the patient is seated in a premium cabin. Alternatively, a full row in Economy Class may be booked for additional comfort.

A medical escort usually flies into Aruba within 24 hours and accompanies the patient bed-to-bed the following day.

Air Ambulance

Air ambulances are traditionally reserved for ICU-level or unstable patients who require continuous advanced monitoring or high flow medical oxygen.

However, because there is no commercial airline stretcher service from Aruba to North America, air ambulances are also dispatched to pick up stable patients who must lie flat, or for patients refused by commercial airlines – For example, those with contagious diseases or severe psychiatric conditions.

An air ambulance from Aruba to Canada typically costs over $80,000 CAD. In certain cases, it may be more financially reasonable to extend hospitalization until the patient is stable enough to travel commercially with a medical escort.

Step 3: Airline Coordination and Logistics on the Ground

Aruba’s Queen Beatrix International Airport is one of the most modern gateways in the Caribbean. The airport has an on-site medical center and is fully accessible. Wheelchair passengers can board aircraft using a so called “ambu-lift”.

There are direct connections to Toronto with WestJet and Air Canada Rouge, as well as one-stop connections to major Canadian cities via the United States, with reputable carriers like Delta, American or United.

But coordination of all parties is of essence.

A smooth medical repatriation involves:

  • Securing airline medical approval in advance
  • Ensuring appropriate medical equipment is available
  • Arranging ground transport in Aruba and in Canada
  • Coordination hospital discharge in Aruba and receiving hospital acceptance in Canada

Attempting to board a flight using improvised or incomplete arrangements can result in last minute boarding refusal by the airline or serious complications. Experience and airline familiarity matter.

How Much Does Medical Repatriation from Aruba to Canada Cost?

Budget is always a critical factor when a medical repatriation to Canada is not covered by insurance.

Costs vary based on many variables and destination city in Canada, but general estimates are:

  • Commercial Medical Escort: anywhere between $8,500 and $28,000 (CAD)
  • Air ambulance: over $80,000.

One of the costliest assumptions families can make is believing an air ambulance is required when it is not. A properly assessed commercial transfer can substantially reduce costs while remaining medically appropriate and comfortable for the patient.

How Long Does a Medical Transfer from Aruba to Canada Take?

Once the patient is stable and paperwork is complete, most transfers from Aruba to Canada can be arranged within 24 to 72 hours, depending on airline availability.

It takes up to 24 hours for the medical escort to arrive on the island. The following day the patient can be on the way to Canada.

A Note About Travel Insurance

If a patient has travel insurance, a delegated assistance company will coordinate the transfer and activate a medical repatriation provider. If there is no insurance — or if families prefer an independent medical repatriation provider — the family can contact a medical repatriation company directly, and a case coordinator can provide guidance throughout the process. Early clarity regarding coverage and fitness to fly prevents unnecessary delays and unexpected expenses.

Important Information for Canadian families:

There are currently no licensed medical escort companies based in Aruba.

If you are approached locally and offered medical repatriation services, you are likely speaking with a broker who refers cases to a provider based in the United States. This introduces an additional layer of commission, communication and liability. Before committing, families should confirm where the medical team is based, whether intermediary fees apply, and who carries medical responsibility for the transfer. Transparency matters, especially in high-stress situations.

Final Thoughts

Medical emergencies abroad are overwhelming. Aruba is a beautiful destination — but not where most Canadians plan to recover long-term from illness or injury.

Medical repatriation isn’t simply booking a flight and hiring a nurse. It requires structured coordination of multiple moving parts and clinical oversight by an experienced transport physician who takes over when the discharging hospital is no longer responsible for the patient.

When managed professionally, the process of medical repatriation unfolds in a controlled and safe way, and far less chaotic way, than families initially fear.

JET COMPANION is a Canadian provider of medical repatriation. The company has developed specific expertise in the retrieval of Canadians on the island of Aruba.
